Family Doctor

Ongoing primary care for patients of all ages — infants, adults, and seniors. We manage both acute and long-term conditions with a focus on prevention and continuity of care.

Private Services & Uninsured Fees

The following services are not covered by OHIP and are billed privately. 

Missed or Late Appointments – $40

Applies to missed appointments, cancellations made with less than 24 hours’ notice, or arriving more than 10 minutes late. This fee must be paid before future appointments can be booked.

Forms and Notes

✦ Therapist Note for Insurance (e.g., Physio/RMT/Orthotics) – $40

✦ Generic Sick Note – $45 (basic signed note, not a specific off-work assessment form)

Procedures

✦ Ear Syringing (for wax removal) – $50

Transferring Reports / Records

✦ Transferring your notes to a physician – $50 (the first 20 pages)

✦ Transferring notes beyond the first 20 pages – $1 per page

✦ Insurance Report (APS – Attending Physician Statement) – $325 (usually paid by your insurer)

✦ Medicolegal Report – $250 (often paid by your lawyer)

Private Services

Consultations and Prescriptions

✦ Private Consultation – $125 (includes visits with an invalid OHIP card or non-OHIP consults such as travel health assessments)

✦ Private Administering of Vaccine – $50 

✦ Private Prescription (including for insurance purposes) – $30 

✦ Other Forms – $125 per 15 minutes (e.g., short-term disability, immigration, or similar forms)

Disability

✦ Disability Tax Credit Form (T2201 – Section B) – $250 (requested by CRA for the disability tax credit)

Employment

✦ Pre-Employment Physical / Assessment / Forms – $225–$250

​

Drivers Medical Requirements

✦ Full Driver’s Physical – $250

✦ Driver’s Medical Form Only – $100

​

Travel

✦ Travel Health Consultation – $125

✦ Travel Certificates - $80

✦ Vaccine Administration - OHIP and Private

Additional Information

Fees may vary based on time and complexity. Payment is due at the time of service unless otherwise arranged. Receipts are provided for insurance reimbursement when applicable.
